I have been doing some work on Endnotes today and in the
process found what appears to a be a major flaw in the mechanism
that fv_View uses to keep track of PieceTable changes. I have
implemented a new mechanism, but thinking that there is a
potential for a major disaster, I put it in place using #ifdef's, so that
it could be easily undone. If it does not have any unforeseen side
effects I will remove the #ifdefs later. (You can find all the changes
searching for INT_PT_STATE.)
