TOC now immediately reflect changes in the heading in the document.

So as you type in in your headings in your document, the text immediately
appears in the TOC. As do changes to the headings.

Next handle some corner cases about inserting the TOC and selecting TOC's.
Then do autonumbering og TOC entries.
